Background Questionnaire

Candidates who are admitted to the Written or Oral Examination must complete a Background Questionnaire before test results are released. 

The questionnaire will take approximately five minutes to complete. You will be required to enter a unique identification number. The identification number is provided to you when you successfully register for an examination. If you have previously registered for an examination, you were provided with your identification number at that time. The identification number is also included in your Official Admission Letter. Complete the questionnaire.
